This guide shows locations of 25 magic totems that give you skill points on first use and buff your signs. The map is in russian but I tried to include recognizable land curves and islands on the screenshots so it shouldn’t be a problem.

This one is in the cave. Character on the map screenshot is standing near the entrance.

Passage to the temple can be seen when standing next to the teleport pole.

Quen

The access to this totem open with the main quest “Bald Mountain”.

Aard

This one is found during a quest with masked elf (Trough Time and Space). You can’t come back to this one after you finish the quest and there is no map so don’t miss it.

Passage to this one is through a broken wall inside the structure the witcher on the screenshot is standing next to and in my case this passage blocked out with an invisible wall after I did the quest in that building. The way around it is up the rocks next to the house as shown in the video.

This one is in the cave on the second level. A ~30 lvl cyclops guards the entrance but you can roll past him. He won’t bother you once you’re inside.

Igni + Aard

One is found during a side quest (Keris) on Skellige (Aard, left on the map).

Another is found during a main quest “Sunstone” (Igni, right on the map).

Kaer Morhen

Igni

The easiest way to access this stone is during the quest for Lambert. Passage to this place of power is through a cave, entrance to which is marked on the map screenshot.
